LICENSE INFORMATION
Section 383 21 FMCSR states 'No person who operates a commercial motor vehicle shall at any tIme have more than one driver's license'. I certify that I do not have more than one motor vehicle license, the information for which is listed below.

DRUG TESTING
Per FMCSA, All CDL drivers operating commercial motor vehicles (CMVs) greater than 26,000 GVWR on public roadways must be DOT drug and alcohol tested. This includes pre-employment, post accident, and periodic drug testing. All applicants must maintain negative testing results and must certify never to operate a commercial motor vehicle under the influence of drugs or alcohol.

• Have a rebuttal statement attached to the alleged erroneous information, if the previous employer(s) and I cannot agree on the accuracy of the information.
